Registration of past births under section D24

1. Such a birth can be registered if it has not been registered within three months of the birth.
2. Submit the declaration to the Additional District Registrar of the Divisional Secretariat.
3. One of the parent or a trustee can submit the declaration form
4. Cost of the Declaration form :- Rs. 50.00
5. Requested documents for late birth registration below one year.
   1. Declaration form (will be provided)
   2. Certified parents' marriage certificate (if married)
   3. Certified parents' birth certificates
   4. One of the below documents to prove date of birth and place of birth
      1. Hospital birth record or certified copy
      2. Grama Niladhari birth report or certified copy
6. Requested documents for late birth registration more than one year.
   1. Declaration form (will be provided)
   2. Certified parents' marriage certificate (if married)
   3. Certified parents' birth certificates
   4. Should be applied for nil result to prove that the birth is not registered.
   5. One of the below documents to prove date of birth and place of birth
      1. Hospital birth record or certified copy
      2. Grama Niladhari birth report or certified copy
7. If the above document is not available, one of the below documents can be submitted
   1. Health Development Chart
   2. Baptized certificate
   3. Certified copy of school attendence register/ school leaving certificate
   4. Horoscope
8. Parents should be appeared to the matter

Issuing probable age certificate under section 24

1. Nil report for proving birth not registered.
2. Hospital record from the hospital of the birth occured.
3. School leaving certificate
4. If the person has a forge certificate that should be proven with an affidavit
5. Grama Niladhari certificate.
6. Parents's Birth certicates and Marriage certificate
7. Person's marriage certificate
8. Children's birth certificate.
9. Copy of National Identity Card
10. According to the office structure should be submitted the certificate from Institute of Forensic Medicine and Toxicology - Srilanka

Requested documents for name change under section D27- Age below 07 years

1. Certified birth certificate to be amended
2. Certified parents' marriage certificate
3. Certified parents' birth certificates
4. If the applicant is father, written consent should be given by mother (Vice versa)
5. Affidavit from JP including that the two names bearing same person. Declarant should be father/mother.(applicant should be signed on Rs.50.00 Stamp)

Requested documents for name change under section D27- Age between 07 to 21 years

1. Certified birth certificate to be amended
2. Certified parents' marriage certificate
3. Certified parents' birth certificates
4. If the applicant is father, written consent should be given by mother (Vice versa)
5. Affidavit from JP including that the two names bearing same person. Declarant should be father/mother.(applicant should be signed on Rs.50.00 Stamp)
6. The relavant name should be used more than one year and it has to be proven with two documents.
7. Relavant name to be changed should be recommend by the letter of Grama Niladhari.
8. If the name need to be changed completely, should be published on a newspaper and the police report is compulsary

Requested documents for name change under section D27- Age above 21 years

1. Certified birth certificate to be amended
2. The relavant name should be used more than one year and it has to be proven by atleast three documents.
3. Relavant name to be changed should be recommend by the letter of Grama Niladhari.
4. If the name need to be changed completely, should be published on a newspaper and the police report is compulsary
5. Affidavit from JP including that the two names bearing same person.(applicant should be signed on Rs.50.00 Stamp)

D51(1) D27(A)- Requested documents to change the other details except name on the birth certificate

1. Certified birth certificate to be amended
2. Certified parents' marriage certificate
3. Certified parents' birth certificates
4. Affidavit from JP including that the two names bearing same person.(applicant should be signed on Rs.50.00 Stamp)
5. Photo copies of parents national Identity Cards
6. Extract of Electoral Register from Election office and certificate on residence issued by Grama Niladhari are essential to amend informant's address.

To get Certified Copies of Birth Marriage and Death Certificates

1. Birth, marriage and death certificates registered up to April 2019 in many divisions in the Colombo District have been scanned and copies can be obtained from our office.
2. Birth, Marriage and Death Certificates registered from 1960 to 2014 in all the districts except Colombo can be obtained from our office.

Death report of hospital/ GN death report / Estate death report

1. Details of bills of good received for the funeral
2. Oath of closest relations
3. GN report
4. Relevant documents to prove that religious activities were done
5. Witness  statements to prove that fund for death were received
